2008 Performance Reservation Form
This is not a contract. This is only for our files that you expressed interest as a performer for the 2008 festival. We will contact you 3-6 months before the event to schedule time slot.
On the performance day, your badges can be picked up at the outside table. They will need to know the group you are with and a valid ID (dirvers license, etc.)
