2007 Martial Arts Festival Parental Consent Form
Dear Parent(s)/ legal guardian(s) of The 2007 Martial Arts Festival staff applicant:
The 2007 Martial Arts Festival will be a convention that brings the Martial Arts in both body and spirit, to local fans. People who come to our convention come from all over the United States and even in countries as far away as Japan and we are in need of volunteers to help us during the convention itself.
Your minor child has expressed interest in becoming a volunteer for The 2007 Martial Arts Festival. Since your child is under 18 years of age, we ask that we have your permission to include your minor as a staff member of The Martial Arts Festival 2007. Please take the time to fill out the bottom portion of this letter for your child to include with his/her Staff Registration Form. With your signature, you will agree to let your minor volunteer for The 2007 Martial Arts Festival.
